2. TERMS OF AGREEMENT

3. PAYMENT
   After you had read and signed the Terms of Agreement, you will be taken to a page with Paypal link where you can pay the fee. 
4. SCHEDULE APPOINTMENT
   After making a  payment, you can book an appointment on the calendar.
